Add assistant role for booking on behalf of doctor

Description
There are one use case where administrative people is scheduling consultation with doctor. This role already exists in backend but it's mostly used as service account to let external application scheduling consultation.
Transform doctor frontend to have this account usable (disable consultation screen, etc...)
