Add Terms acceptance for Doctor:

• Add new parameter "DOCTOR_TERMS_VERSION" in Backend.
• If DOCTOR_TERMS_VERSION is 0 or unset, terms will not be shown.
• If DOCTOR_TERMS_VERSION is integer more than 0 (e.g. 1), Terms acceptance will be shown right after the login (new page).
• Tickbox must be enabled and doctor must click on "Accept" button.
• The doctor profile is then updated with the terms DOCTOR_TERMS_VERSION into a new field.
• If DOCTOR_TERMS_VERSION is updated and doesn't correspond to new DB field value, show again the terms.
