Description

Have a new settings called ENABLED_FIELDS_FORM_DOCTOR, used to override what patient information are required. When unset, keep the default "Firstname", "Lastname", "Sex".

This settings will be useful to only enable "Firstname" and "Lastname" while sex is then hidden.
